In clinical studies 97% of patients showed a visible improvement after just one treatment with Belotero. Belotero plumps the skin and smooths away lines and wrinkles in the periorbital and perioral areas of the face- with possible supplemental “touch-up” treatments required to achieve and maintain the desired look. Is Belotero the same thing as BOTOX? While BOTOX and Belotero are both effective at reducing wrinkle treatments, how they work differs. What is BOTOX Cosmetic? BOTOX® Cosmetic is a purified protein (neuromodulator) used to block nerve impulses that tell your facial muscles to contract. The result is that it creates a temporary relaxation of these muscles that cause wrinkles from frowning, raising your eyebrows, and squinting. Once it is injected, the muscles will be very limited in how much they can move. By reducing this movement, you can no longer make the facial expressions that cause the wrinkles to appear, and the skin becomes smoother. What Is the Difference between BOTOX and Facial Fillers? Botox and fillers are complementary procedures – but they are not the same thing and should not be thought of the same. Though they are both injections, they act quite differently. Botox is a neuromodulator which when injected in to target muscles Botox relaxes the lines that are created by the muscle movement such as the “11” in the frown area (forehead between the eyebrows). Treatment will last 3-4 months and with the right technique and skills, a patient can look more youthful without any of the usual telltale signs of a Botox treatment. Fillers are just that – they “fill in” the creases and wrinkles that are present at rest, such as the eye hollows, smile folds, and jowl lines. The majority of fillers are made of Hyaluronic Acid and can also be injected into the deeper dermal skin layer to restore lost volume and shape to the face. These thicker fillers are used to plump up depressed cheeks, temples and jowls. What is the Skin MD and Beyond Difference? It also has no effect on sensory nerves, so you will have normal feeling in the treated areas. What is the treatment like? A. A tiny amount of BOTOX® Cosmetic is injected very precisely, into the muscle being treated. Because the needle is so fine and only a small amount of liquid is used, the pain associated with the injections is minimal. Most patients compare the sensation to a mild bug bite. No sedation or local anesyhetic is required, and you can resume normal activities immediately. How long does BOTOX Cosmetic treatment last? A. The effects of BOTOX® Cosmetic usually last three to four months, at which time another injection is required. After several treatments, the effects of BOTOX® Cosmetic may begin to last longer. What is DYSPORT? Dysport is a medically derived from botulinum toxin called botulinum type A – the same derivative as Botox. Dysport received its approval from the FDA in 2009. Like Botox, it works by reducing the movement of specific, “wrinkle-causing” facial muscles which means that it weakens the signal from the nerve to the muscle into which it is placed. The precise placement of Dysport into specific areas of the face, blocks these signals from the nerves, allowing the muscles to relax and smoothing the wrinkles that they cause. In experienced hands Dysport can provide a very natural appearing and dramatic improvement in your appearance. Enjoy the Experience! What is the Skin MD and Beyond Difference? Fiona Wright MD was privileged to be one of the first physicians in the Dallas-Fort Worth Metroplex to offer Dysport to her patients. Having vast experience with injectable facial contouring, news anchor, Meredith Land, interviewed Dr. Wright for the NBC Evening News at SKIN MD & Beyond to learn the latest about Dysport. What Is the Difference between BOTOX and DYSPORT? Overall Botox and Dysport are very similar. Both are a type of botulinum type A but are made by different companies. Both are used to relax the muscles that cause frown lines and other facial lines caused by active facial muscles. Both have been in use for years and have a high safety profile, both can be used on other parts of the face and body, and both Dysport and Botox must be injected every 3-6 months. The main difference between Dysport and Botox is in the amount of proteins that surround the active molecule. Dysport has fewer proteins and is an overall smaller size particle. Because of this difference, Dysport results will take effect sooner and the transition between treated and untreated areas more subtle and less noticeable. This can help create a more natural look after the procedure. There has been much discussion amongst physicians about the differences between Botox and Dysport, or IF there is any difference. Here are the main differences: Dysport may not last quite as long (duration of efficacy). This is still being debated and studied. Dysport has a slightly quicker onset (2-5 days vs 4-7 days for Botox) Dysport diffuses more (i.e., spreads to a wider area). This is an advantage in some areas of the face and a disadvantage in others. It is good in that large areas that need injection (forehead, armpits) would require fewer injections due to the spreading. With fewer injections you can have less potential discomfort, swelling or bruising. The disadvantage is that unless the Physician is an experienced injector with a good understanding of more complex musculature (around the eyes, between the eyebrows), the Dysport could spread into unwanted areas causing untoward side effects (blurry vision, droopy eyebrows). You need more Dysport to get the same results as Botox. Dysport and Botox “units” are not the same, so the amount needed to treat an area varies greatly. The general rule is that you will need 2.5-3 unit of Dysport for every 1 unit of Botox used. Drs. Wright have vast experience in treating patients with both Botox and Dysport and during your consultation they will help you decide which treatment would be best suited for your individual goals. In some instances, Dysport may be desired for its ability to “diffuse more”. But in other instances, Botox will be a better choice for precise placement in small areas of concern. The main thing when receiving injections is to make sure the person injecting is well versed and skilled in injecting whichever product they are putting in your face. What Is Juvéderm and Juvéderm Plus? Juvéderm products are North America’s most popular facial fillers! Juvéderm and Juvéderm Plus products are a “soft” clear gel filler with extraordinary flow properties which assures a very natural-feeling and natural-looking result. It is derived from a hyaluronic acid substance (non-animal stabilized hyaluronic acid (NASHA)). The substance is naturally known to aid the skin in maintaining proper hydration and volume. What Is the Difference between Juvéderm, Juvéderm Plus and other Facial Fillers? There are several hyaluronic acid fillers on the market -the most well known being Restylane and Juvéderm. Juvéderm is quite similar to Restylane chemically, but has some differences in the structure. Restylane is composed of small gel particles which gives more lifting power, so they can be injected into deeper tissue and provide better results on deep folds. Juvéderm is a softer gel that is easier to inject and causes less tissue irritation. Juvéderm Plus is a Juvéderm variation that is thicker. Both Juvéderm and Juvéderm Plus are a clear colorless gel and has the smoothest consistency available on the market today, designed to produce the desirable results that last as long as possible. How long does a treatment usually take? The injection process is quick and easy and takes only about 15-30 minutes, depending on the areas treated. Patients are asked to come in 30 minutes prior to their appointment for proper cleansing and prepping of the skin and topical numbing application. Additional time is also considered to allow time to consult with your doctor prior to treatment. How long does Juvéderm last? The unique formulation of Juvéderm has been clinically proven to last for six months to a year. Because Juvéderm Plus is a thicker product it can last several months longer. However, we are all individuals and I have seen patients occasionally where it only lasts 3 months and others where it has lasted up to two years – but these individuals are the exception, not the rule. How long before I see results? You will see a visible improvement immediately after treatment. Most people need one treatment to achieve an optimal outcome. Additional “touch-up” treatments may be required to achieve and maintain the desired look. Dr. Wright will work with you in the room to achieve the most desirable results. The best results are gaged after about 2 weeks. How is Juvéderm different than Botox cosmetic? Juvéderm™ injectable gel is a dermal filler that provides volume and is used to treat wrinkles and folds resulting from a loss of volume beneath the surface of the skin such as perioral lines and nasolabial folds (lines around your mouth and nose). Juvéderm restores you’re the volume in your lips and kin for a smooth, natural look and feel. BOTOX® Cosmetic is not a filler. BOTOX® Cosmetic relaxes muscle activity and is used to treat moderate to severe lines caused by muscle movement. As the muscle movement softens there is a temporary diminishing of lines for a smoother appearance. Can people with different skin tones be treated with Juvederm? Yes. In fact, Juvéderm injectable gel is the first FDA-approved hyaluronic acid dermal filler that has proven its safety and effectiveness in persons of color. Studies with Juvéderm showed no increased risk of hyperpigmentation or hypertrophic scarring in patients of color. Am I a candidate for treatment with Juvéderm? Do not use this product if you have severe or multiple allergies or a history of severe allergic reactions manifesting as anaphylaxis. You should also not use this product if you have allergies to lidocaine or the proteins used to make hyaluronic acid. Dr. Wright will determine if Juvéderm treatment is right for you. Juvéderm Injectable Gel contains trace amounts of gram positive bacterial proteins and is contraindicated for patients with a history of allergies to such material. Does a Juvéderm treatment hurt? In most cases, not at all. The area is numbed with topical anesthesia and once it is absorbed, you will barely feel the Juvéderm injections. What is KYBELLA? KYBELLA is the first and only FDA-approved, non-surgical treatment that removes your double chin by reducing the fat under the chin without surgery. Kybella contains a naturally-occurring ingredient called deoxycholic acid, something your body already produces. This ingredient destroys fat cells. Afterwards, those cells can no longer store or accumulate fat. For years, liposuction or surgery were the only options to eliminate the double chin. However, patients wishing to melt chin fat without surgery can now do so with Kybella. Who can benefit from KYBELLA? KYBELLA will work for anyone with fullness below the chin. Anyone wishing to reduce their double chin without surgery or liposuction is an ideal candidate for non-surgical Kybella. The best candidates have elastic skin (ie are young enough that the skin will spring back). The drug does not tighten the skin, but there may be some skin contraction and some patients in clinical trials have reported tighter skin under the chin. However, individuals with sagging skin may notice more looseness afterward and may require a skin tightening procedure such as radiofrequency or a face lift. It is not a treatment for weight loss. For those who have a double chin due to obesity, diet and exercise are the recommended solutions. How quickly can I see results with KYBELLA? Patients should plan on about two to four KYBELLA treatments spaced one month apart. Each treatment will be about 15-20 minutes in length. Many patients have seen reduced chin fat in two treatments, but four to six may be required for fuller chins and necks. These results may last for several years and re-treatment is often not necessary. What are the possible side effects? KYBELLA has been approved by the FDA and is considered safe. In the clinical trial of 5000 patients, the most common side effects were swelling at the injection site, bruising, pain, temporary numbness, reddening of the skin, and areas of firmness. The most serious side effect was injury to a branch of the facial nerve in the jaw area, causing a crooked smile. This occurred when injections were given too close to the nerve. The problem occurred in 4% of participants in the trial and eventually resolved in everyone. How long is the recovery? There is some inflammation and tenderness after treatment which may last 3-5days. Since this is a non-invasive procedure, the recovery time is faster than liposuction and is reported by most patients to be just a few days. How does RESTYLANE and RESTYLANE SILK differ from other Facial Fillers? The family of Juvederm Facial Fillers are also a very popular, and there are some subtle differences Between the two types of facial fillers; Juvéderm has a higher concentration of hyaluronic acid and attracts more fluid, which makes it an ideal choice for volumizing lips but not for subtle lip contouring or for under eye correction. Restylane and Restylane Silk are best used for the tear troughs (area underneath the lower eyelids). Juvéderm is a little more expensive than Restylane, and a syringe of Juvéderm contains less product than Restylane (they contain 27% less filler than Restylane syringes), so Restylane may be preferred by some patients. Juvéderm tends to feel softer so it tends to provide a smoother result and not as likely to be lumpy. On the other hand, however, it doesn’t seem to provide the lift that Restylane provide. To achieve “full volume correction”, you need to replace enough of the lost volume to get a great result. So, for deeper wrinkles or to add volume Voluma, Radiesse, and Restylane LYFT are better products for volumization. How long does RESTYLANE / RESTYLANE SILK last? One of the great advantages of Restylane is that the effects are long lasting but not permanent. Duration depends on many factors, such as the individual’s skin type, lifestyle and age. After the initial treatment, follow-up sessions are normally recommended around every 6 to 12 months depending on the individual and the treatment. Some clinical studies have shown that if you are treated again at 4 1⁄2 to 9 months after your first treatment, Restylane can last up to 18 months in the nasolabial folds. In the lips, Restylane generally lasts six months. How long before I see results? You can expect instant results upon injection. The doctors can see the correction changes evolve during your treatment. They will also give you a mirror so you can see the effects and add a little more filler if you would like. What Is the Difference between RESTYLANE LYFT and other Facial Fillers? The biggest difference in the various fillers used by Drs. Wright at Skin MD and Beyond, is what they are made of. Restylane, Restylane LYFT, Juvéderm, and Belotero are hyaluronic acids or sugar-derived molecules which retain water like a sponge, adding volume to the treated areas. They are all clear and gel-based. Restylane LYFT is thicker than the other above fillers and is meant for deeper injection. Even though all of these products are hyaluronic acids, the “G prime” or “stiffness” of Restylane LYFT is much higher and Restylane LYFT can support a “heavier load” and create a better lift in facial contours. To lift up and contour the face, Drs. Wright choose to use Restylane LYFT, Voluma and Radiesse. Radiesse is a calcium-based filler, that is white and thicker, while, Voluma is the thickest and most robust hyaluronic acid available. Because of the thickness of the latter two products, they need to be placed deeper in the skin and are not ideal or intended for use in the lips or under the eyes. For first time patients, Restylane LYFT or Voluma is often chosen because it is, “kinder and gentler” the first few days. The Radiesse tends to have more swelling during this time. The advantage of the Radiesse is that, for many, Radiesse lasts longer and can “lift” more than Restylane LYFT, and is less expensive then Voluma. These horizontal wrinkles are caused by contraction of the frontalis muscle which is an elevator of the eye brow. Small injections using a very fine needle are strategically placed in the forehead, and relaxation of this muscle occurs. This results in significant improvement in forehead wrinkles. When strategically injected in the forehead, the muscles relax and lines that worsen with movement are less likely to form and the lines will soften. It is important to recognize that if the lines are present at rest, the treatment may not completely smooth these lines. While Botox and Dysport are most commonly used to reduce the lines of the forehead (by reducing the muscle movement that cause the lines), Facial Fillers can be used in combination to instantly help soften the appearance of depressed lines etched in the skin. Frown lines / vertical forehead lines / or the “elevens” between your eyebrows are lines that develop over the years as we contract the muscles that make us concentrate, frown or squint. Over time, these lines etch in permanently and are there even when the person is not frowning. This is one of the most requested Botox treatments at Skin MD and Beyond, as these lines make a person look mad, tired and unapproachable. Dysport is a botulinum toxin similar to that of Botox and is also commonly requested to reduce the appearance of wrinkles and frown lines. When Botox/Dysport are placed correctly the effect will be a relaxed, calm and refreshed brow that provides an excellent result! If your lines are deeper, and prominent without movement, the combination of Botox and a Facial Filler is even better and lasts longer. The two are synergistic and are able to get rid of frown lines much better when combined. The frown area between the brows looks the way it does because of muscle movement making the lines, volume loss and the effects of aging. Botox/Dysport is a temporary fix to block the muscles for a 3-6month period. This keeps the muscles from creating deep wrinkles in the area, but it doesn’t improve the volume loss. Fillers are a great way to improve the lines that form in this area. By placing filler in the “cracks” it makes it harder for the muscle to actually create the wrinkles. Drs. Wright are very experienced in treating the frown lines with a combination of Botox/Dysport and Fillers to correct both issues. Ideally, the Botox/Dysport may be injected ten days before the filler, or in patients who are unable to return for a second appointment, the two may be injected at the same time. The two in combination work better than either alone. Amazingly in many patients the vertical line doesn’t return even after the Botox has worn off. Treating the frown lines with fillers is considered and “off-label” use and you should always ensure that your doctor is very experienced in treating this area. Only a conservative amount of filler can be used to avoid extra swelling that may compromise blood flow. There are reports of complications from fillers injected which has caused tissue death of small patches of skin. When the filler is correctly applied (i.e. not too much)this rare complication can be minimized.

Squint and Nasal Lines Nasal Flare Nose Tip Lift Crooked Nose – Non Surgical Nose Job SQUINT AND NASAL LINES: Recommended treatments Botox If you scrunch up your face and squint (the procerus muscle), you see the lines that form on the nose bridge (scrunch lines) and the ones on the side of the nose which are known by the unscientific term “bunny lines.” Some of us make this expression more than others, but almost all of us get these lines as we age. Botox is a great treatment for bunny lines. Usually with a small injection on either side of your nose, the bunny lines can be significantly reduced and the overlying skin will become smoother and the lines less prominent – however, they are seldom eliminated. The idea is to “weaken” the muscles which bunch the skin to make the bunny line wrinkles, and it is usually advantageous to have some residual activity to maintain normal-appearing animation. It is important to ensure that your treatment should only be performed by a doctor that is very familiar with facial anatomy as this is an off label usage and requires a higher level of knowledge and experience. If the Botox if placed incorrectly (too much or too wide apart), the muscles that lift the corners of the mouth (levator labii superioris muscle), could be affected which would make the mouth look crooked, or like a stroke occurred. This involves careful placement of high concentrations chemical called TCA (trichloroacetic acid) into the scar. LARGE PORES and BLACKHEADS – Plano and Frisco TX Two of the most stubborn and hard-to-treat skin-care problems are blackheads and enlarged pores. Simply put, they’re tough to eliminate completely. What Are Blackheads? In general a closed comedone is a whitehead, while an open comedone is called a blackhead. Blackheads or whiteheads are generally caused by blockage of the sebaceous or oil glands found near the surface of the skin and excrete an oily substance known as sebum. Surprisingly, the visible black head isn’t caused by dirt, but by the sebum mixing with dead skin cells and oxidizing when it hits the air. Blackheads are those little black spots that appear around the nose, chin and foreheads – and anywhere else on the body they wish to rear their ugly “heads”. The oil producing glands are called sebaceous glands and are very sensitive to hormonal changes specially during puberty, pregnancy, menstruation and can lead to an increase in the production of sebum and an increased risk of blackheads. The sebaceous glands produce too much oil in the pores, which accumulates and gets stuck. When the occluded oil is exposed to air it becomes black. It is believed blackheads appear at the initial stage of acne appearance and form before bacteria penetrates the patient’s skin. A blackhead can subsequently develop into a pimple, known as a papule or pustule. The good news is that getting rid of blackheads is easy through proper skin cleansing and extraction methods. It’s important to know that you can’t actually “shrink” your pores. They’re nothing but small openings in the skin — because they don’t have muscles, they can’t open or close. So although you can’t physically change the structure of your pores by making them smaller, there are things that you can do to minimize their appearance. The causes of red skin can be a mystery for some. The most common causes for adults are Rosacea Sun-damage Eczema and Sensitive/Sensitized Skin. Rosacea is a very common skin concern, particularly in women with lighter skin types. It is characterized by an excessive amount of redness in the skin, due to dilated blood vessels and traditionally it is a problem for those with sensitive skin and cannot tolerate regular anti-aging products or anything with harsh chemicals. (see section on Rosacea for a complete description). Sun-Damage is a common problem in Texas and our faces show the most damage. With continuous sun exposure and lack of SPF sun protection, many people will notice unsightly telangiectasia (small threadlike blood vessels) that are harmless but can create a burning or stinging sensation and will make the skin quite sensitive. (see section on Sun Damaged Skin for a complete description) Eczema has become two or three times more common in recent decades. It is more common in children and usually resolves before adolescence, but 2 to 10 percent of adults suffer with eczema lifelong. Eczema usually presents with dry skin that can become red, and will have scaly patches that can be quite itchy. The skin of people with eczema seems to be sensitive to many substances, including wool fabrics, laundry detergent residues, and certain cosmetic. (see section on Eczema for a complete description) Sensitive/ Sensitized Skin is triggered by external factors (pollution in the air, chemicals in our water, aggressive weather conditions, cosmetic ingredients and poor diet) that results in the breakdown of our skin’s protective barrier. As a result, facial skin becomes dehydrated and red – and looks very similar to the three previous mentioned conditions. Is your skin sensitive or sensitized? Having truly sensitive skin is in your DNA and is a genetic predisposition where the skin is thin and blood vessels are close to the surface. A person with sensitive skin is prone to asthma and allergies, and eczema. Sensitized skin is not a type but a condition brought on by environmental triggers like pollutants and chemicals in the atmosphere, and from irritants found in many cosmetics and skin products, from harsh scrubs, and from improper cleansing habits at home. Sensitized Skin is in fact a growing concern and is becoming a common reason why patients are seeking help for their skin care concerns. It can affect any age or race, and any skin type, from dry skin to acne. (see section on Sensitive/Sensitized Skin for a complete description)
Melasma, also known as hormonal pigmentation, is a considerable problem for Texan women, as the sun’s UVA rays are a stimulant for melanin (brown pigmentation) to develop on the skin, and unfortunately the sun can even penetrate through car window glass. It is a chronic skin condition that occurs on the face in the form of brown patches and is a dynamic (active) condition that needs lifelong management. It generally appears on the cheeks, forehead, nose and around the mouth, but it could turn up anywhere. The cause is a combination of hormones and the sun; no matter how much sun protection you use, you could still get it. The pigment cells are also heat sensitive, so if you go out in very warm weather, you’re at risk. It’s more common in pregnant women because they have high estrogen states, but it can happen to anyone. Any change in hormonal status with higher levels of estrogen is thought to trigger and aggravate the condition (i.e. birth control pills and pregnancy). Discontinuing the use of contraceptives rarely clears the pigmentation and it may last for many years after discontinuation. But, after pregnancy Melasma or pigmentation will usually clear within a few months of delivery but may it may also persist indefinitely. However many women without these risk factors also develop melasma. Melasma is seen most frequently in young women but can be seen in men. It is primarily induced and exacerbated by sun exposure, but may also be triggered by; Hormonal therapy such as oral contraceptive use or hormone replacement therapy Pregnancy Endocrinologic disorders such as Thyroid disease Medications such as Dilantin and Minocin Cosmetic Skin Care Any activities that cause Intense Heat to the face (from steam showers, hot closed environments, sitting on the beach even under cover and with full sun protection) There are two types of Melasma and each describes how deep the pigment is in the skin and how to treat it. In the office Dr. Wright will evaluate the depth of pigment with a Wood’s lamp, which can roughly estimate if pigment is in the upper layers of skin and will be amenable to topical therapy with hydroquinone and retinoids, or if it will require a deeper treatment with a laser, light source or microneedling. As melasma has a tendency to reoccur (particularly in darker skin tones) consistent use of a retinoid and photoprotection can help prevent flares. Epidermal Melasma is the most common form and is easily treated with a combination of superficial chemical peels, IPL light base procedures. Dermal Melasma occurs when the melanin has leached into the deeper skin layer called the dermis. The deeper the pigment is -the more difficult it is to treat. Dermal melasma is not treated with superficial peels and is best treated with lasers and microneedling.
Milia are small white bumps that form on the face when dead skin cells get trapped beneath the skin’s surface, forming small, hard cysts as a result. Milia are commonly found on the skin of people of all ages, but it is most commonly seen in infants and are affectionately called “milk bumps” – although they have nothing to do with milk. Nearly all babies and most adults who suffer with milia get them because the skin has not been properly exfoliated. Adults can also experience secondary milia, however, which develop after something clogs the ducts leading to the skin surface, such as after an injury, burn, or blistering of the skin. Unlike blackheads and whiteheads, milia do not develop into acne lesions. Milia may disappear without treatment. In fact, when infants have milia, most doctors will recommend that you do not attempt any special treatment. Merely wash your baby’s skin with warm water and pat it dry with a soft towel every day. In adults, milia appear as 1–2 mm white-to-yellow, dome-shaped bumps that are not painful or itchy. There are usually located on the faces of people who have had a lot of damage from sun exposure and are most commonly found around the eyes, cheeks, nose, and forehead. How to Treat Milia and Clogged Pores in Adults At Skin MD and Beyond, there are many ways to remove milia and Dr. Wright with her vast experience in treating them, will provide you with the best option – for the best cosmetic outcome. Resist the temptation to scratch them away. Milia are not pimples, so popping them will not be able to make them go away. The hard cyst lying under your skin will remain there unless a professional can lance it. You are unlikely to have success attempting its removal and will likely cause irritation and damage to your skin. You may even create a scar. Milia are safely removed with a milia extractor which is a simple procedure that we perform almost every day in our clinic. For those that do not have a milia extractor, a simple unroofing of the cyst with a sterile scalpel blade or needle, followed by the use of a comedone extractor can also be used. It should heal with minimal-to-no scarring and for best results it should be performed by a professional with a lot of experience in milia extraction. Because milia can form when the skin’s natural exfoliation process malfunctions, to prevent further milia: consider switching cosmetics, moisturizers and/or sunscreens, and practice sun avoidance. You can use a targeted exfoliating treatment to immediately improve exfoliation and allow milia to dissolve before they start.
At some time in our life, we have all had a concerning spot show up on our skin, or may have had a multitude of marks that appear on the surface of the skin, particularly the face. Some concerns may be very innocent (skin tags, milia, seborrheic keratosis, warts) and can be removed for cosmetic reasons only. See information at Forehead: Lumps, Bumps and Moles. Others spots, although not cancerous, may still need to be removed for medical reasons. See information Cheek: Lumps, Bumps and Moles. But the most important spots that require close attention are moles. Most people have moles. They appear as brown ‘spots’ of varying sizes on the skin. They can be flat or raised, big or small. Usually they are quite round but can sometimes be irregular in shape. Medically moles are called ‘naevi’ (or nevi) with one mole being a ‘naevus’ (or nevus). Moles can be present at birth and very often more develop as people age, normally as a result of exposure to the sun. Moles are a collection of pigment containing cells called melanocytes within the skin and their presence should not cause any concern or serious problems. However, some moles can go on to develop one of two conditions called ‘non malignant melanoma’ and ‘malignant melanoma’. It is the signs that show that these moles might be changing that we all need to be aware of. What are the Signs that a Mole is Changing? The border may become irregular, uneven or blurred There may be a change in colour An increase or decrease in size and elevation It could be itchy or painful Bleeding Everybody should examine their moles on a regular basis to look for any changes to moles that are already there or the appearance of new ones, particularly if there is a family history of skin cancer. Non-melanoma skin malignancies are quite common, affecting up to 1 in 6 of the population, predominantly in those who have had sun exposure during their lives (sun-bathing, outdoors sport, gardening, living in hot countries etc.)
Molluscum Contagiosum is a common skin disease caused by a virus which harmless flesh tone or pink raised bumps. At first, the molluscum spots look like pimples. They then become round, pearl colored lumps that have a white mark or core in the center. Usually they appear on the underwear area, stomach, face, arms and legs. Sometimes the molluscum may look like other skin condition, such as warts, chickenpox, and even skin cancer. Molluscum is contagious and can be spread from person to person or to different areas in the same person. It can also be spread by sharing towels and clothing. Most people get about 10 to 20 bumps on their skin. If a person has a weakened immune system, many bumps often appear. Usually it does not need any treatment. Good personal hygiene can help prevent it spreading. Most of the time, Molluscum does not need any treatment. The spots usually disappear over a period of time as the body fights and kills the virus. A small number of people can have the spots for several months or years. Aggressive treatment may cause scarring and for this reason, it is best to try simple treatments first. Simple treatments are designed to irritate the spots. This is to encourage the body’s immune system to recognize the virus and destroy the infected cells.
Skin tags are common, benign skin growths that look like a small soft balloon, tab or “flap” of hanging skin that usually appear on areas where there is skin friction. The most common places are on the neck, under the breast and underarms, and between the inner thighs. Skin tags are harmless growths that can vary in number from one to hundreds. Males and females are equally prone to developing skin tags. Skin tags are thought to be hereditary or acquired over time. Increased body weight and hormonal elevations such as pregnancy can also contribute. While most tags typically are small (2 mm-5 mm in diameter) at approximately one-third to one-half the size of a pinky fingernail, some skin tags may become as large as a big grape (1 cm in diameter) or a fig (5 cm in diameter). Skin tags can be easily removed in the office. Local anesthesia is used to make the removal painless. There are several effective medical ways to remove a skin tag, including removing with scissors, freezing (using liquid nitrogen), and burning (using medical electric cautery at the physician’s office).

Many patients tell us that they have “sensitive skin” when in fact, what they are really suffering from is a case of “sensitized skin”. Although the behaviors of these two conditions can present the same, there are some unique and crucial differences between the two. Sensitive Skin is an inherited genetic trait, meaning you were born with it. It is a specific skin type that cannot be biologically changed. Steps can be taken to make your skin less sensitive – but general strategies will always be required to keep your complexion as calm as it can be. Sensitized Skin is far more common and it is acquired through lifestyle choices that have made the skin more sensitive. It is not a skin type, but a skin condition that all skin types (dry, oily, normal) can be affected by. By removing the triggers and making better lifestyle choices, your complexion can return to its normal state as it is not a permanent condition. Both skin “states” are similar in that the protective mantle of the Stratum Corneum (the outermost layer of skin) has been thinned out or damaged. When this protective barrier (epidermal lipid barrier layer) has been compromised micro cracks in the skin appear which allows for our valuable water content to evaporate quickly and allows unwanted pollutants, irritants and microorganisms to enter into the deeper layers of our skin. When this happens the typical signs are dry, flaky, cracked, irritated, itchy or uncomfortable skin. What are the Skin Symptoms of Sensitive and Sensitized Skin? Both conditions present the same, as both are a result of a compromised/damaged epidermal lipid barrier. The difference is that with sensitized skin, these symptoms go away once the skin has repaired itself and with naturally sensitive skin, these symptoms are often ongoing The most common finding are: Tingling, stinging, prickling, or burning Hot or warm Red and blotchy Blushing or flushing Dry, tight, flaky, or itchy Dry patches, cracked skin Spots and Bumps Sensitive to touch React quickly to heat (sun, hot water, high temperatures) Easily irritated to products or changes
If your skin is more than dry, with red, scaly patches that itch like crazy, you may have a condition called Atopic Eczema, also known as Atopic Dermatitis or simply Eczema. Though it’s a chronic condition you may have to deal with indefinitely, there are ways to keep eczema symptoms under control and avoid future outbreaks. Atopic eczema is the most common form of dermatitis, or skin inflammation, affecting as many as 10 to 20 percent of people around the world. It’s called atopic — meaning that it is allergy related — because certain types of exposure seem to cause the rash. The skin of people with atopic eczema seems to be sensitive to many substances that cause the eczema reaction, including wool fabrics, laundry detergent residues, and certain cosmetic ingredients. Although probably not the true cause of eczema, certain foods may make atopic eczema worse for some people, and eliminating them from the diet may help improve skin. Atopic eczema can strike anyone from babies to the elderly, and most often appears for the first time at a very young age — generally before age 5 and often before age 1. It usually starts as an itchy, red patch of skin that may blister, ooze, and crust over. Finally, a thick patch of skin may appear where the rash has healed. Atopic eczema tends to run in families and it is most common in childhood. There is no cure but most cases can be controlled by applying various creams to rehydrate the skin. 60-70% of cases resolve by adolescence. Treating Eczema Eczema treatment can be complex and comprises of a maintenance skin care program to reduce the incidence of flares as well as treatment for the acute episodes. Treatments should focus on the repair and maintenance of the skin barrier, control of inflammation, reduction of allergen exposure and reducing itch. Dr. Wright is ideally qualified to discuss with you how to control your eczema in a totally safe manner which will not harm the skin as well as discuss with you the various treatment options and put this all in perspective for you. Management is achieved by use of anti-histamines, barrier repair moisturizing creams (such as CeraVe®), and prescription anti-inflammatory corticosteroid creams and ointments. For severe cases, drugs such as cyclosporine A or Imuran may be recommended. The Treatment Plan for Eczema consists of Three Parts: Avoiding irritants to the skin and other causes (triggers) wherever possible.
